How do I register my child at Desert Star Academy?

Click the link below to download the enrollment form for the school in which you wish to enroll your child.

1. Print out the forms (If you do not have printer access you may pick up the enrollment forms at our school office)
2. Complete the Form(s).
3. Bring your completed enrollment form(s) to the school office, located at 5744 Highway 95, Fort Mohave, AZ 86426

School Office Hours

Summer hours: Mon-Thu 8:00am - 2:00pm
Regular hours: Mon-Fri 7:30am - 3:30pm
